Registration Information


All forms must be filled out in their entirety before your child can attend the Arlington Summer Blast day camp.  To open these forms you will need to use Adobe Acrobat - a free download available at http://get.adobe.com/reader/. If you are unable to open these forms, please contact the Camp Office at 410.655.9300 x.144.

Referral Discounts: Referral discounts of 10% are applied to the Program Fees when referred new campers and referring campers reserve and attend the same week. A referral form must be filled out by both parties in order to receive the discount. The referral form is available here: http://www.arlingtonbaptistschool.org/summerblast/files/referral2010.pdf.

Medications: Maryland state law requires any camper needing any form of medication (prescription or off the shelf) is required to submit medication in its original packaging along with a signed Physician’s Medication Order Form (PMOF) by the camper’s physician to the camp director. This also includes any lotions or sunscreens that need to be applied during day camp hours. The PMOF is available here: http://www.arlingtonsbaptistschool.org/summerblast/files/pmof.pdf
